In order to register at the surgery it will be helpful if you have one or more of the following:
- Passport
- Birth certificate/marriage licence
- UK driving licence
- Rental agreement/rent book
- Utility bill
- Bank statement
We might not be able to register with you if you live outside our practice boundary.
